Welcome to this Active Learning Module (ALM). This ALM is funded by the Australian Government through the Australian General Practice Network (AGPN) and was developed by experts from the Baker IDI Heart and Diabetes Institute.
This ALM aims to help general practitioners improve patient outcomes for those at risk of developing type 2 diabetes mellitus (T2DM). This training program will provide information to assist with the identification of type 2 diabetes, or signs of pre diabetes, while also outlining the benefits of Lifestyle Modification Programs (LMP).
The program outlines the eligibility criteria for referral to a Lifestyle Modification Program and also explores a range of techniques that could be used to motivate high risk patients to attend an LMP.
Research has found that interventions such as these can prevent or delay the onset of diabetes by up to 58%*.

The Prevention of Type 2 Diabetes: Identification and management of high-risk patients online interactive training program adds a new dimension of training options, providing an alternative to face to face training for busy service providers.

The AGPN acknowledges the financial support for the 'Prevention of Type 2 Diabetes' program from the Australian Government Department of Health and Ageing.
This ALM has been approved for the 2011-2013 RACGP triennium. To obtain 40 category 1 QI & CPD points you must complete the three modules within the ALM. Each module is 2 hours in length. Module 1 of the program is a prerequisite to the completion of further modules. The ALM has been approved with ACRRM.
